Senior Care Program Supervisor

Oversee the Home at Last Program as a Supervisor at CHATS-Community & Home Assistance to Seniors. This position offers an opportunity to lead a team dedicated to senior care and support.

As Supervisor, you will be responsible for the daily operations of the Home at Last Program, which facilitates transitions for seniors from hospitals back home. You'll manage scheduling, referrals, and collaborate with various healthcare partners. Strong organizational skills and the ability to lead teams are essential for this role.

Key Responsibilities:

• Supervise daily operations and staff management

• Coordinate referrals and scheduling for senior clients

• Foster relationships with healthcare facilities and partners

• Monitor program quality and effectiveness

• Ensure compliance with relevant laws and organization policies

Requirements:

• Post-secondary education in relevant fields

• Minimum 3 years of supervisory experience

• Strong leadership, communication, and coaching skills

• Valid Ontario Driver's Licence and reliable transportation

• Capacity to manage competing priorities effectively
