Full Time, 80.00 hours per pay period (2 weeks)

Shift: Evenings
Pay: $31.61 - $47.41 / hour. Pay is dependent on applicant's relevant experience.
This position is an onsite role and does not offer a hybrid or remote option.

Summary

Assists with Investigations, assessments and intervention plans to help patients cope with social, emotional, economic and environmental problems. Operates as a co-responder with local area first response agencies.

Responsibilities
  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
  • Determines, provides and/or arranges for needed internal and external services/interventions. Participates and/or arranges case reviews/care conferences to evaluate progress. Consults with healthcare team members to promote, monitor and evaluate compliance with patient's treatment plan.
  • Responds in the field as a co-responder with local law enforcement to patients in active crisis. Identifies appropriate resources including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the field response.
  • Utilizes appropriate teaching methods and/or referrals to ensure patient/family educational needs are met. Participates and/or trains new staff, students and interns as assigned.
  • Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
Requirements
  • Master's degree in Social Service or Behavioral Health related field.
  • State licensure as a Licensed Professional Counselor Candidate (LPC‑C) or a Marriage and Family Therapist Candidate (MFT‑C) or Licensed Addiction Counselor (ACD), Social Work Candidate (SWC) or State licensure as Licensed Social Worker (LSW).
  • Preferred: 2 years of relevant experience.
