• Provide emotional support and companionship to residents, addressing their social, emotional, and spiritual needs
• Coordinate with interdisciplinary teams to develop and implement individualized care plans
• Facilitate resident engagement in activities, outings, and events, promoting socialization and overall well-being
• Assist with daily living tasks, such as personal care, grooming, and meal assistance
• Maintain accurate and detailed records of resident interactions, needs, and progress
• Collaborate with family members and caregivers to ensure seamless communication and support
• Participate in staff training and education programs to stay updated on best practices and regulatory requirements
