Pharmacy Technician
The Pharmacy Medication Transition Technician assists inpatient and outpatient pharmacists with medication history, reconciliation, to ensure that any differences between medications taken before admission and medications ordered upon discharge for a patient are intentional and not accidental. Works to promote an efficient discharge prescription process in collaboration with the outpatient pharmacy.

- Shift Times:
Monday - Friday, 8am-8pm. Saturday, 8am-5pm and Sunday, 9am-2pm. - Shift Length:
Varies between 5-10 hours. - Weekend Requirement:

- Data Entry:
Med Transitions technicians obtain insurance information, if needed, add patient demographics, insurance information, and prescription data into the outpatient pharmacy prescription processing system. Technicians then submit the claims to third party insurance for processing, which will determine co-pay and whether adjudication issues may exist. If adjudication issues exist, the technician will troubleshoot and begin the resolution process. - Patient Contact:
Med Transitions technician communicates with patient or patients' families information on co-pay amount and any insurance adjudication issues. Technician will also verify patient payment type. - Product Dispensing:
Medication transition technicians coordinate dispensing and patient education activities with the decentralized pharmacists.
- Education:
High School Diploma or Equivalent - Experience:
Two (2) years of Pharmacy Technician Experience. Medication Reconciliation Technician or Meds to Beds experience is preferred. - Equivalency:
- Completion of pharmacy technician program may substitute for one (1) year of the experience requirement.
- Completion of pharmacy technician program plus one (1) year of experience performing medication prior authorizations may substitute for two (2) years of pharmacy technician experience.
- Certification(s):
- State of Colorado Pharmacy Technician (PHAT) Certification
- Certificate through a nationally recognized certification board or body approved by the National Association of Boards of Pharmacy (NABP) such as the Pharmacy Technician Certification Board (PTCB) or National Healthcare Association (NHA)
- Medication History Certificate within 12 months of hire

Pay is dependent on applicant's relevant experience.
Hourly Range: $23.54 to $35.31
