Job Summary And Qualifications
The Registered Nurse (RN) Case Manager (CM) is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team.
The RN CM coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. Provides case management services for both inpatient and observation patients as assigned.
- Identify patients at risk for adverse outcomes during transitions between levels of care or settings.
- Perform comprehensive assessments of psychosocial, medical, and discharge needs for patients and families, and assess available resources.
- Reassess the patient’s clinical condition as indicated and develop strategies to mitigate readmission risk, including education on accessing healthcare resources and community-based supports.
- Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary health care team and the patient’s physician.
- Partner with Social Services/Social Work to ensure post-acute needs and level of care are appropriate; refer promptly when risks are identified.
- Involve patients and family members in identifying needs and setting mutual, realistic goals.
- Evaluate progression of care using evidence-based tools and approved criteria (Inter Qual) and manage progression and transition issues through the chain of command.
- Make referrals to third-party payers and disease/case management programs for patients with recurring or chronic conditions.
- Facilitate patient throughput with a focus on effective care transitions, quality, and efficiency.
- Document professional recommendations, discharge plans, and coordination activities for communication to the health care team.
- Align patient needs with available resources to ensure a safe discharge/transition.
- Act as a liaison among physicians, patients/families, hospital staff, and outside agencies.
- Identify opportunities to control costs without compromising patient safety and care quality; address needs of under-resourced patients through education and community resources.
- Participate in performance improvement activities and escalate avoidable days as appropriate.
- Adhere to policy, procedures, standards of care, and the established chain of command; stay informed on regulatory requirements and quality initiatives.
- Associate Degree in Nursing or Nursing Diploma Required
- Bachelor’s Degree in Nursing Preferred
- 2+ years of experience in case management OR 3+ years of experience in clinical nursing required
- Inter Qual experience preferred
