Job Summary

Part‑time Clinician (Nurse Practitioner or Physician Assistant) providing in‑home health evaluations for Signify Health members. Visits last ~45 minutes and include medication and medical history review, physical assessment, and diagnostic tests as ordered by the member’s health plan. Clinicians use an iPad and clinic workflow during each visit.

• Build trusted relationships to improve health outcomes and connect members to appropriate care.
• Visit members nationwide with a goal of delivering personalized evaluations.
• Complete assessment procedures, including medication history review, physical exam, and diagnostic testing per plan requirements.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Offer Signify Health services such as spirometry, diabetic eye exams, and other preventive tests.

Clinicians work in the private residence of health plan members. Must be comfortable working safely in homes that may contain pets, secondhand smoke, or other allergens.

Annual hours: 29.5. Time type:
Part time. Pay range: $41.85 – $90.13 per hour. Base salary depends on experience, education, geography, and other factors.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state license(s) preferred.
• Ability to cover Grand Junction/Mesa County.
