Sweetwater Care - Nurse Practitioners, we are searching for you! Our multistate long term care facilitator is seeking a Nurse Practitioner to steward the health of residents amongst two to three skilled nursing facilites in Colorado. The ideal applicant will be willing to visit each facility weekly to assist in assessing current interventions, ultimately to facilitate the reduction of hospitalizations and improving overall wellbeing of those we care for.
Candidates will be responsible for working in conjunction with providers to facilitate comprehensive care to the residents we serve. Applicants should have significant experience in geratrics, long term or skilled care and possess strong leadership skills. This is postion that will require facility presence with the Nurse Practioners as active members of the Interdicplinary Teams. Hours would be Monday through Friday 8-4, no on call provisions needed.
- Active and unrestricted Registered Nurse and Nurse Practitioner license in the state of Colorado
- Up to 75% travel required amongst the assigned facilities
- Board certified by ANCC or AANP.
- Current and unrestricted DEA certificate.
- Ability to work without direct supervision and practice autonomously.
- Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology.
- Ability to communicate effectively, in verbal and written form, partners at various levels, patients, family members, physicians and representatives of the community.
- Sound understanding of all federal and state regulations including HIPAA and OSHA.
- Minimum of 2 years of experience as an NP working in an SNF or geriatric setting
- Strong background in patient assessment, diagnosis, treatment, and management of disease processes.
- Excellent communication skills, works well in a team environment
