Job Summary

Children's Hospital Lifestyle Medicine Tier 2 (LM2) Program provides comprehensive health care integrated with research study participation to children, adolescents and young adults with obesity and obesity-related comorbidities including type 2 diabetes, PCOS, hypertension, MASLD, dyslipidemia, sleep apnea, and more. The main focus of this position is to provide initial evaluation and care of patients with newly diagnosed T2D as well as new and follow-up care to patients in our LM2 clinic.

This is a collaborative team role with physicians, NPs, certified diabetes educators, nurses, dietitians, psychologists, social workers and pharmacists to improve health disparities for youth with obesity and related comorbidities.

Key Responsibilities
  • Triage referrals for pre-adolescents/adolescents/young adults (approximately 8-21 years) in the LM2 EPIC pool from LM1 and on-call Pediatric Endocrine providers to coordinate scheduling of initial care and follow-up for LM2 patients. Assess severity, urgency, and fit for the LM2 clinic.
  • Perform detailed history, physical exam and formulate differential diagnosis for new-onset diabetes. Include education on medications, injections, and glucose monitoring.
  • Coordinate follow-up care for adolescents/young adults (12-21) with newly diagnosed T2D with medical providers and CDE.
  • Perform follow-up clinical visits in a multidisciplinary clinic for patients with T2D, including medical treatment, referral for bariatric surgery as appropriate, and screening/treatment of comorbidities.
  • Perform clinical visits in a multidisciplinary clinic for other insulin resistance-associated conditions (hypertriglyceridemia, PCOS) with medical treatment and comorbidity screening.
  • Assess home glucose monitoring and medications between visits to adjust diabetes and weight loss regimens.
  • Outpatient follow-up visits for babies with hypoglycemia related to stress hyperinsulinism; coordinate safety visits for medication discontinuation.
  • Outpatient follow-up for secondary adrenal insufficiency: monitor steroid tapers, adrenal recovery, glycemia; provide stress dose teaching; coordinate ACTH testing visits.
  • Establish systematic follow-up to evaluate plan of care, reassess and modify as needed; initiate referrals and consultations.
  • Adequate patient/family teaching for home management; promote self-care and informed decision making.
  • Collaborate with program CDE/RN to standardize diabetes educational materials and streamline clinical processes (quality improvement).

Qualifications

Minimum Qualifications:

Applicants must meet minimum qualifications at the time of hire.

  • Master's degree in nursing;
    Registered Nurse license and Certified as a Family Practice or Pediatric Nurse Practitioner; or Physician Assistant (PA) degree.
  • Current license to practice as a nurse practitioner or physician assistant in the State of Colorado, USA.
  • 1-year experience in direct patient care.

Condition of Employment:

  • Must successfully pass a drug test through Children's Hospital Colorado.
  • Must be willing and able to pass a national criminal background check.
