In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Montezuma

Job Summary

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves meeting patients in their homes to conduct clinical assessments and facilitate care transitions.

• Conduct 45‑minute in‑home evaluations, including medication and medical history review, physical assessment, and order‑based diagnostic tests (e.g., spirometry, diabetic eye exams).
• Use an iPad and a clinical workflow system to document encounters.
• Bridge gaps in care by connecting patients with appropriate services and follow‑ups.
• Provide diagnostic & preventive services as indicated.
• Travel locally within licensed states and conduct virtual visits as needed.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• For NP and PA positions, mastery of clinical practice in the field.

• Multi‑state licenses.
• Experience working in home‑based care environments.

Clinicians work in patients’ private residences, which may include household pets, second‑hand smoke, and potential allergens or irritants. Candidates must be able to perform duties safely in these settings.

Pay Range: $41.85 - $90.13 per hour (base rate depends on experience, education, and location).
