In-Home Health Clinician - Nurse Practitioner or Physician Assistant Grand Junction

Overview

Signify Health, part of CVS Health®, is hiring a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, helping members receive comprehensive care in the comfort of their own homes.

• Conduct 45‑minute in‑home visits, including medication and medical history review, physical evaluation, and any ordered diagnostic tests.
• Use an iPad and intuitive clinical workflow for each evaluation.
• Build and maintain trusted relationships with patients to improve health outcomes.
• Travel locally and statewide as needed, and conduct virtual visits when required.
• Provide Diagnostic & Preventive Services such as spirometry, diabetic eye exams, bone density screening, and other indicated tests.
• Follow the company’s safety and quality standards while working in diverse home settings.

• Active, unrestricted NP or PA license(s) in the licensed area(s).
• Board certification (required); multi‑state license preferred.
• Experience delivering in‑home or virtual patient care.
• Ability to travel within the Grand Junction MSA and adjacent counties.
• Comfortable working in homes that may contain pets, secondhand smoke, or allergens.

Typical base pay range: $87,035.00 – $ annually, dependent on experience, education, and location.
