Job DetailsWe are expanding our in‑house wound care program to serve home‑bound patients with chronic wounds who cannot access a brick‑and‑mortar wound clinic. The program serves patients in assisted‑living communities and private residences. Wound care providers perform comprehensive assessments and create evidence‑based treatment plans tailored to individual goals.
We are currently looking for a Nurse Practitioner to join our Wound Care Team!
Qualifications- Master's degree required
- Licensed as a Nurse Practitioner in Colorado
- Wound certification preferred
- Deliver wound care during home visits to 7‑8 patients per day
- Implement evidence‑based wound care interventions
- Determine appropriate follow‑up frequency and manage ongoing caseload
- Demonstrate exceptional communication and relationship‑building skills with care team members, internal clinicians, and external partners
- Utilize population health data to drive superior patient outcomes
- Participate in ongoing educational opportunities
- Commute to assigned assisted‑living communities and patients’ private homes to provide care
- Scheduled days:
Monday‑Friday (5 days) - Relocation assistance packages available
- Flexible schedules with weekends off
- 7‑8 visits per day

The salary for this position is competitive and commensurate with experience. The pay range for this role in Colorado typically falls between $100,000 - $115,000 annually with potential for performance‑based bonuses and other benefits. Actual compensation may vary based on qualifications, experience, and location within the state.
