** Position Summary** :

Responsible for providing services to both inpatients and outpatients, including assessment, treatment, planning, implementation and related documentation.

** Position Requirements** :

+ Graduate of approved School of Occupational Therapy.

+ Licensed as an Occupational Therapist in the State of Illinois.

+ Holds and maintains requirements for credential of "Registered" as issued by NBCOT

+ Certified in CPR through the American Heart Association.
