Shawnee Health is seeking a
Patient Care Coordinator II
for our Shawnee Health office. This position reports directly to the Patient Care Coordinator Supervisor.
Responsibilities
- Schedule medical referrals, diagnostic tests and treatments ordered by provider and assure that the patient has been notified, understands directions, and completes all documentation in the patient’s Electronic Medical Records.
- Reviews and selects medical records pertinent to referrals, diagnostic tests and treatments to be sent with order to provider or facility
- Completes the pre-authorizations process with insurance companies as it relates to the referral and/or diagnostic test scheduled.
- Follows up on referrals and diagnostic tests ordered to ensure completion and the receipt of all reports including consult notes and test results.
- Notify provider of referrals or tests that have not been completed by patient.
- Works to complete EHR task as assigned
- Works directly with the social service team assisting with the identification of patients with special needs including but not limited to: financial assistance, transportation, pharmaceutical assistance, interpretation, or special accommodations.
- Completes prior authorization process for referrals and diagnostic testing when required.
- Assist Patient Care Coordinator Supervisor with training new employees.
- Monitors access issues with referrals and provides consistent feedback to Patient Care Coordinator Supervisor
Must be competent in the use of computers
Must be capable of independent problem solving
Must be able to work with little direct supervision
Must have excellent written and verbal communication skills
Starting salary – $16.00
Full-time employee benefits include:
