Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this classification positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The CCHIP Sexual Health Clinic Coordinator will provide patient centered HIV counseling, testing, PrEP education, assessments, and referrals for initiation to adults and young adults (adolescents) in the Sexual Health Clinic, community settings and in any Cook County Health (CCH) clinic where sexual health and PrEP services are provided, under direction of the Manager of Patient Centered Care (Manager). Develops, implements, and provides patient education in related topics (i.e., PrEP, HIV, STIs, hepatitis C, medication adherence, etc.).
Maintains a linkage database and submits monthly reports of program activities to the Manager, funders, and other administrative staff. Participates in all administrative meetings and staff development meetings, representing health education and prevention issues, activities, goals, and outcomes.
This is a grant-funded position. The Grant 2026 CCH Ryan White Part C Outpatient grant will expire on July 31, 2026, with the potential to be renewed.
TYPICAL DUTIES
- Provides patient centered HIV counseling, testing, HIV, STI, PrEP, & DoxyPEP education, assessments, and referrals for initiation to adults and young adults (adolescents) in the Sexual Health Clinic, community settings and in any CCH clinic where sexual health and PrEP services are provided.
- Provides patient education on HIV transmission, adherence, treatment, and other diseases such as hepatitis, dietary needs, risk reduction, diabetes, hypertension, and other health related topics in collaboration with the Manager.
- Coordinates and monitors PrEP related care and services.
- Supervises the health education staff that provide sexual health services
- Assesses individual and group health education needs in the Sexual Health Clinic and in community settings.
- Maintains current knowledge of HIV and other Sexually Transmitted Infections (STI's) related to its etiology, transmission, and treatment. Additional topics include Pre-Exposure Prophylaxis(PrEP), risk reduction, nutrition, diabetes, hypertension, hepatitis, and Partner Services (PS).
- Provide linkage to HIV/HCV care, PrEP services, medication management, adherence, and treatment. Conducts outreach/home visits as needed to link patients to HIV services.
- Provides assistance with linking patients who are HIV positive on the Stroger campus and the Emergency Department and as referred by Cook County Health (CCH) and community providers to their patient-centered model home (PCMH).
- Provides post-test HIV counseling in the Stroger ED (adult and PEDS) and other CCHIP sites as referred by providers and at community testing events.
- Conducts outreach services for prevention, treatment, and linkage to care in the community.
- Works in collaboration with the nursing staff to manage Sexual Health clinic operations.
- Documents of all face to face, phone calls, and collateral contacts in the EMR (Electronic Medical Record) and data base (CAREWare/PROVIDE).
- Updates and maintains policies and procedures for the Sexual Health clinic.
- Completes departmental reporting, including PrEP monthly report-CDOH and Prevention & Education report.
- Assists with development of peer educators and integrated care specialist.
- Reviews grant related budgets.
- Co-facilitates the monthly sexual health meetings.
- Performs other duties as assigned.
Minimum Qualifications
- Bachelor's degree from an accredited college or university, is required (Official transcripts are required at time of interview)
- Two (2) years of HIV counseling experience, is required
- One (1) year of experience supervising or managing staff, is required
- HIV Testing and Counseling certification, is required (Must present certificate at time of interview)
- Proficiency using Microsoft Office (i.e., Excel, Word, and PowerPoint), is required
- Position requires moderate local travel for which the employee must possess a valid driver’s license and insured vehicle or otherwise provide an acceptable and reliable means of transportation, is required
- Must be flexible to work evening and/or weekend hours, is required
Preferred Qualifications
- Bachelor's degree or higher in a health-related field, education, psychology, social science from an accredited college or university, is preferred.
- Partner Services experience and certification, is preferred.
