Center Well Home Health is seeking a Home Health Occupational Therapist to help patients regain strength, mobility and independence in their homes. You’ll assess daily living and work-related skills, design therapeutic retraining programs, and work with physicians and the health care team to optimize the Plan of Care.
You will document care observations, participate in care coordination and discharge planning, and design adaptive equipment or training for patients and families.
