Case Manager Registered Nurse - Field; Kane and LaSalle Counties

• Pay: $66,575 - $142,576/year

• Residency:
  Candidate must reside in Kane, LaSalle or surrounding counties
• Travel:
  Willing and able to travel up to 50-75% of the time in Kane, LaSalle and other surrounding counties

• Facilitate the delivery of appropriate benefits and healthcare information
• Develop, implement, and support health strategies and policies
• Act as a liaison with members, employers, providers, and insurance companies
• Implement and coordinate case management activities for catastrophic cases
• Assess and analyze medical and vocational status of members
• Communicate with members and stakeholders telephonically or in person
• Prepare required documentation of case work activities
• Consult with internal multidisciplinary teams to maximize health outcomes
• Provide educational and prevention information for best medical outcomes
• Monitor member progress toward desired outcomes through assessment and evaluation

• Associate’s Degree in Nursing (REQUIRED)
• Bachelor’s Degree in Nursing (PREFERRED)

• 3+ years clinical practical experience
• Preferred: 2+ years case management, discharge planning, and/or home health care coordination experience

• Active and unrestricted Registered Nurse (RN) in Illinois
