Case Manager – Collaborative Diversion

Salary $30.77/HR - 1 year grant funded position, with possible extension

Hours Full-time – Non-Exempt (35 hours/wk.)

The case manager position involves serving clients in the greater Aurora area under the umbrella of Kane County Collaborative Diversion. Collaborative Diversion is a system of response that replaces the traditional pipeline of punishment with long‑term, patient, non‑coercive, and non‑judgmental case management for low‑level, nonviolent offenders.

• Initiate and schedule regular contact times with participants in order to assist in meeting goals
• Maintain accurate digital records for all participant forms and documents
• Meet with Operations Workgroup as scheduled to provide participant updates and collaborate on participant care.
• Advocate for participants within the criminal legal system as necessary and appropriate
• Identify systemic barriers and potential solutions to address the needs of participants
• Attend supervision meetings with Case Manager Supervisor
• Foster positive relationships with law enforcement partners, local service providers, and other key stakeholders in the county and criminal justice system.
• Use of the harm reduction methods
• Referrals to appropriate service providers
• Adhere to confidentiality and privacy policies and refrain from divulging any identifying information about participants.

• Minimum high school diploma/GED plus four years' experience in social services or harm reduction OR a four-year degree in related social sciences (social work, psychology, sociology)
• Cultural competency
• Understanding of harm reduction model
• Experience working with highly complex populations, individuals that suffer from mental health issues, substance abuse disorder.
• Comfort in highly variable settings.
• Client rapport, empathy, compassion, active listening
• Ability to build and maintain client relationships
• Conduct threat assessments when meeting clients to ensure safety
• Case management skills
• Organization, record keeping, information technology savvy, healthy professional boundaries.
• Valid Illinois Driver's License

• Driving – local travel required
• Ability to lift up to 15 pounds on occasion
• Sitting, standing, talking, hearing and vision required approximately 100% of the time
• Ability to speak loud and clear
