Our experienced and dedicated team assist the residents transition into a long‑term care facility and continue to work with them on any challenges they may be facing, such as anxiety, depression, and other mental health issues.
Our Licensed Social Workers and Counselors deliver personalized services within the nursing home and/or assisted living facility, ensuring that each client's unique mental health needs are met with compassion and expertise.
- Active Ohio license as a Licensed Independent Social Worker (LISW)
or Licensed Professional Clinical Counselor (LPCC) - Passion for working with the elderly population
- Ability to work independently within a facility setting
- Strong communication and clinical documentation skills
1. Evaluating Clients:
- Conduct thorough assessments to understand clients’ needs, challenges, and mental health conditions.
- Identify individualized support options based on these evaluations.
- Develop customized care plans for each client, considering their unique circumstances and mental health requirements.
- Work closely with healthcare professionals, nursing staff, and case managers to coordinate services effectively.
- Facilitate seamless communication and holistic care for clients.
- Regularly assess clients’ progress and adjust care plans as needed.
- Keep track of treatment outcomes and make necessary modifications.
- Document treatments, interventions, and services provided.
- Ensure accurate and confidential record-keeping.
- Engage in therapeutic conversations with clients to address emotional and behavioral issues.
