Position Summary

CarePoint is seeking a dedicated and clinically skilled Nurse Practitioner (NP) specializing in wound care to join our team. The NP will provide on-site wound care services in skilled nursing facilities, working collaboratively with interdisciplinary teams to assess, treat, and monitor patients with acute and chronic wounds.

This is an excellent opportunity for a provider who values autonomy, flexibility, and making a direct impact on patient outcomes.

Key Responsibilities
  • Perform comprehensive wound assessments and develop individualized care plans
  • Provide treatment for acute and chronic wounds (pressure ulcers, diabetic wounds, surgical wounds, etc.)
  • Perform bedside procedures including debridement (as appropriate based on training and scope)
  • Collaborate with physicians, nursing staff, and facility leadership to ensure continuity of care
  • Educate staff, patients, and families on wound care prevention and treatment
  • Document all encounters accurately and timely in EMR systems
  • Monitor wound healing progress and adjust treatment plans as needede
  • Ensure compliance with all regulatory and clinical standards
Qualifications
  • Board Certified (FNP, AGNP, or related specialty)
  • Wound care experience preferred (SNF or long-term care a plus)
  • Wound Care Certification (WCC, CWCN, etc.) preferred or willingness to obtain
  • Strong clinical assessment and decision-making skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage a flexible schedule
