The Service Coordinator partners with residents to promote resident wellness. They continually assess and assist residents to access services necessary to remain in independent living setting. They maintain complete list of service agencies. They motivate residents to actively participate in problem solving, planning for their health, safety and social well-being. They maintain records and report regularly to HSM and HUD. They present or procure programs for residents' continuing education on numerous pertinent topics.

They coordinate community food assistance programs.

Education

Bachelors in Social Work, Healthcare, Gerontology - Preferred

Experience

2-3 Years - Required

Social services;
Health services, computer

License & Certification

None Required

Core Job Functions

  • Evaluates and assists residents with mental, physical and emotional health needs. Checks on residents daily, weekly, monthly or quarterly as needed and responds to emergency needs with appropriate direction. Keep informed of local service organizations to connect residents to the services as needed.
  • Responds to emergency call system, evaluates residents' situation, calls for emergency services as needed and discuss situation with families for needed support/services. Connect residents to services as needed. Responds to smoke alarms as needed, and evaluates safety and well-being of residents.
  • Uses AASC Software to manage residents' confidential records in accordance with HUD requirements and updates records annually and as changes occur. Meets HUD reporting requirements.
  • Provides educational programs for residents to help maintain health and well-being. Coordinates meal programs and distribution, allocates resources meals and brown bags.
