The RN Care Coordinator manages transitional care for hospitalized patients, assessing needs, coordinating care, and facilitating discharge planning.
Responsibilities- Conduct psychosocial and functional assessments
- Collaborate with multidisciplinary teams
- Address barriers to discharge and ensure timely patient progression
- Develop individualized discharge plans
- Manage post-acute referrals
- Document all interactions accurately in medical records
- Engage in professional development, mentorship, and adherence to policies to support organisational goals
- At least one year of acute care nursing experience
- RN licensure (single or multi-state)
- BLS certification
- Relevant education (Associate, Diploma, or Bachelor's Nursing preferred)
- Must demonstrate maturity, confidence, and the ability to work under stress in a fast‑paced environment, collaborating effectively with physicians, patients, and families
- Strong communication, organisational, and problem‑solving skills
- Focus on patient‑centred care and resource utilisation
