Qualifications And Skills

- RN license (single-state or multi-state compact) and BLS certification
- Minimum of 1 year nursing experience in acute care
- Excellent communication, assessment, organizational, and problem-solving skills
- Knowledge of healthcare regulations, community resources, and care coordination best practices
Conduct assessments, manage discharge planning, facilitate interdisciplinary communication, resolve care delays, and ensure documentation accuracy. Support patient-centered care by respecting cultural and religious preferences and collaborating effectively with the care team to promote smooth transitions and resource utilization.
