Role:
Hospice admission RN, a licensed registered nurse responsible for educating patients and families about hospice services, performing admissions, and developing initial care plans.
Key Responsibilities:- Conduct assessments to determine hospice eligibility
- Explain admission criteria and benefits
- Coordinate resources for home care
- Obtain physician orders
- Create personalized care plans in collaboration with the interdisciplinary team
- Provide emotional support during the transition to hospice
- Review medical histories and medications
- Educate families on care procedures and after-hours support
- Associates or Bachelor's degree in Nursing
- RN license (single state or multi-state compact)
- BLS certification
- Valid driver's license
- Private vehicle insurance
Skills:
- Minimum of 2 years in acute care and hospice preferred
- Knowledge of nursing practices
- Excellent communication
- Patient advocacy are essential
- Must adhere to professional standards, policies, and ethical guidelines
