Regency Home Health Care is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team and provide high‑quality care to our clients in their homes. As a home health care provider, you will have the opportunity to work one‑on‑one with clients with high medical needs, including ventilator/trach care, in a private home setting. ALL SHIFTS available!

• Work hands‑on with medically complex clients, advocating for their physical and mental well-being
• Perform assessments and monitoring, providing assessment details and information to supervisors and doctors
• Perform medical treatments under the direction of doctors
• Schedule, attend, and follow up on medical appointments
• Order and track inventory of supplies, equipment, and medication for clients
• Help maintain a positive and professional environment
• Assist coworkers as needed and promote teamwork

• Hold current RN/LPN license in the state of Minnesota
• Hold current CPR certification
• High school diploma or equivalent required; experience in home health care/public health, trach, G‑tube, and/or ventilator experience highly preferred
• Demonstrate consistent integrity, good judgment, and initiative

• Provide effective and efficient care to clients with high medical needs
• Develop and maintain positive relationships with clients, families, and healthcare teams
• Ensure accurate and timely documentation of client data and care plans
• Collaborate with other healthcare professionals to provide comprehensive care
• Identify and report any safety concerns or issues related to client care

Competitive hourly rate: $27.50-$43.00/hour (dependent on experience and licensure)

RN: $38.00-$41.00/hour

LPN: $28.75-$29.25/hour
