Positive Support Analyst

As a Positive Support Analyst at Sevita, you'll empower individuals of all ages and abilities to live more independently in their homes and communities. You will deliver person-centered positive supports, implement behavior and skill-building strategies, and promote inclusion and safety. Working closely with clinicians, families, and community partners, you'll document progress, assist in accessing resources, and help prevent crises through proactive support.

Responsibilities

• Provide person-centered positive support and coaching to individuals in home and community settings
• Implement support and behavior strategies that promote independence, safety, and inclusion
• Document services, progress notes, and incident reports accurately and on time
• Collaborate with clinicians, families, and community partners to align support plans
• Assist individuals with accessing community resources, activities, and services
• Monitor progress toward goals and share updates with the care team
• Support crisis prevention and de-escalation using approved techniques
• Advocate for individuals' preferences, rights, and self-determination
• Participate in ongoing training, supervision, and team meetings
• Model Sevita's values of empathy, respect, and collaboration in all interactions
