Job Summary: The Relief Habilitation Coordinator works alongside a team of advocates to help adults with Intellectual and Developmental Disabilities (IDD) seeking admission to a Medicaid‑certified nursing facility, who may also have a co‑occurring mental health diagnosis, to ensure access and continuity of care. The coordinator follows the federally mandated Preadmission Screening and Resident Review (PASRR) program guidelines, uses state electronic systems such as CARE and TMHP, facilitates transition duties, and applies a person‑centered approach with four service coordination elements: assessment, service planning/coordination, monitoring, and crisis prevention/management.
Regular, predictable attendance is essential; a flexible work schedule may be required.
- Education: Bachelor’s degree from an accredited college or university with a major specialization in Human/Behavioral/Social Services or a related field.
- License: Current and valid driver’s license and a clean driving record.
- Experience: At least one year of experience working directly with individuals with DD.
Hours: Full‑Time.
Workdays: Monday – Friday.
Workhours: 8:00 AM – 5:00 PM.
Salary: $24.18–$25.39 an hour based on education.
