Overview
The Crisis Specialist QMHP is responsible for ensuring the provision of recovery-focused, person-centered care with a trauma-informed perspective. This position operates in a dynamic environment, and performs case management, assessment, and/or service coordination work based on the program's guidelines. The QMHP's duties include providing continuity of care services, linking consumers and families to needed resources, coordinating, monitoring, and updating services and consumer goals.
Additional roles may include providing skills training, crisis prevention assistance, and community outreach. As a team member of programs in crisis services, the QMHP coordinates crisis intervention tools to address client needs using assessment, multidisciplinary coordination, and efficient clinically appropriate use of agency resources. Duties include collaboration with consumers, first responders, and community stakeholders to address immediate mental health crisis and plan for future crisis relapse prevention.
