Job Summary
The Case Manager is a Qualified Mental Health Professional (QMHP) who provides case management, service coordination, and continuity of care for diverse individuals involved in the criminal justice system. Working in collaboration with community partners, this role conducts assessments, develops service plans, and links individuals and families to behavioral health, medical, housing, and other community resources. The Case Manager provides psychosocial rehabilitative skills training, facilitates evidence-based interventions, supports crisis prevention and intervention efforts, and coordinates multidisciplinary responses to stabilize individuals and reduce relapse risk.
This position partners closely with Integral Care staff, first responders, and community stakeholders to ensure appropriate use of resources and continuity of care. The role operates in a dynamic environment under clinical supervision and may require flexible scheduling, community-based work, and bilingual service delivery as needed.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education- Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.
- Current and valid driver's license and a clean driving record.
Full-Time.
WorkdaysMonday - Friday.
Workhours8:00AM - 5:00PM.
Salary$24.18 - $25.39 an hour, based on education plus an additional 10% crisis stipend.
