Job Title: Community Health Worker (Guia)

Location: Austin, TX (North Austin)

Compensation: $25 – $30 / hour

Job Description

The Community Health Worker serves as a key member of the clinic care team, supporting a dedicated patient panel. This role focuses on building meaningful relationships with patients and their families, helping them navigate the healthcare system, and encouraging proactive participation in their health journey. The community health worker partners closely with providers, medical assistants, nurses, and care coordinators to help patients meet preventative, chronic, and acute care goals.

Key Responsibilities
  • Manage and support an assigned panel of patients to ensure care plan adherence and follow-up
  • Provide education and tools that empower patients to make informed health and lifestyle decisions
  • Identify and help address social determinants of health impacting patient outcomes
  • Collaborate with a multidisciplinary clinical team to coordinate care efforts
  • Build rapport with patients during clinic visits and community interactions
  • Document patient interactions accurately within electronic systems
