Overview

Work at the Austin State Supported Living Center, a facility that provides campus‑based direct services and supports to people with intellectual and developmental disabilities.

Key Responsibilities
  • Perform routine consultative services and provide technical assistance during monitoring of individuals following their move from a State Supported Living Center (SSLC) to a community setting.
  • Assist in the development of supports and services as part of the transitioning planning process.
  • Observe and interact with individuals and community provider staff to determine if the supports and services identified in the transition plan are in place and effective.
  • Complete written reports and notify SSLC staff of any identified issues.
  • Mediates disputes or dissatisfaction with community supports and services following transition and works to develop solutions for identified issues.
  • Schedule, coordinate, and conduct on‑site monitoring at 7, 45, and 90‑day intervals and as needed.
  • Complete monitoring checklists after each visit and communicate results to the Admission/Placement Coordinator and interdisciplinary team.
  • Participate in disaster response, recovery, or COOP activation when required.
  • May serve on a rotating on‑call schedule to meet operational needs.
Knowledge, Skills & Abilities
  • Ability to prepare reports and communicate effectively orally and in writing.
  • Ability to work professionally with individuals with intellectual disabilities, family members, staff, community providers, and others.
  • Knowledge of HHSC community programs including ICF/IID and waiver programs.
  • Skill to gather, assemble, correlate, and analyze facts and devise solutions to problems.
  • Computer proficiency in Word, Excel, and other Microsoft Office products.
  • Ability to handle multiple responsibilities and plan work activities.
  • Willingness and ability to travel up to 80% of the time.
Qualifications
  • Bachelor’s degree, preferably in a human services field, or one (1) year of work experience in quality assurance, program monitoring, service coordination, case management, or providing services in a health care field.
  • Experience working in community Immediate Care Facilities/Intellectual Disability (ICF/IID) or Home Community‑based Services (HCS) waiver programs preferred.
  • Experience working with individuals with intellectual or physical disabilities preferred.
  • Willingness to travel up to 80% of the time.
Additional Requirements
  • Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including CANRS, Nurse Aide, Medication Aide and Employee Misconduct and HHS LEIE.
  • Males aged 18-25 must be registered with the Selective Service.
  • All State Supported Living Center employees are subject to random drug testing.
  • Flexibility in work hours; overtime, extended hours, weekends and holidays may be required.
  • All applicants must be at least 18 years of age to be considered for employment.

Telework:
This position is not eligible for telework. The position may be eligible for telework per state and agency telework policies, subject to supervisor discretion and business needs.
