Speech-Language Pathologist (SLP) – PRN

**** Pay Rate:** $60.00–$65.00 per hour

Join Our Rehabilitation Team Medical Hill Healthcare Center is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our therapy team on a PRN basis. In this role, you will help residents improve their communication, cognitive-linguistic, and swallowing abilities while collaborating with an interdisciplinary team dedicated to delivering exceptional patient care.

Key Responsibilities Resident Evaluation & Treatment
* Assess residents to identify communication, cognitive, and swallowing deficits
* Develop and implement individualized treatment plans based on physician orders and clinical findings
* Monitor resident progress and modify treatment plans as appropriate

Interdisciplinary Collaboration
* Partner with physicians, nursing staff, families, and other healthcare professionals to coordinate care and optimize outcomes
* Participate in care planning meetings and discharge planning discussions

Education & Training
* Educate residents and caregivers on treatment goals, home programs, communication strategies, and swallowing safety
* Provide in-services and educational support to staff as needed
* Mentor Clinical Fellows (CFY), students, and rehabilitation team members when applicable

Documentation & Compliance
* Complete timely and accurate documentation in accordance with facility policies, regulatory standards, and payer requirements
* Maintain compliance with all federal, state, and facility guidelines

Quality & Safety
* Promote a safe treatment environment through adherence to infection control and safety procedures
* Support quality assurance and performance improvement initiatives Qualifications
* Current State Speech-Language Pathologist (SLP) license
* Certificate of Clinical Competence (CCC-SLP) preferred
* Experience in skilled nursing, post-acute care, or geriatric rehabilitation preferred
* Strong communication, organizational, and teamwork skills
* Commitment to providing high-quality, resident-centered care
