Job Summary
As a paraprofessional member of the interdisciplinary group, works under the supervision of a registered nurse and performs various services for a patient, as necessary, to meet the patient’s personal needs and to promote comfort. Responsible for observing the patient, reporting these observations, and documenting observations and care performed.
What you will do in this role
- Performs personal care needs for assigned patient(s) and attends to his/her requests promptly. If unable to perform a certain task, report to the Hospice Director or Director of Clinical Services immediately
- Observes the patient and reports observations; documents observations and care performed
- Provides patient and family with positive communication techniques within the realm of ethical and respectful care, including confidentiality
Qualifications
- Driver License (DL)
- High School Graduate / GED
- No travel required
- No prior experience required

Hospice & Family Care provides physical, emotional, and spiritual support for patients and families. We help navigate each step of the hospice journey. Our care teams include physicians, nurses, therapists, hospice aides, social workers, music therapists, spiritual counselors, volunteers, and bereavement specialists. Regular scheduled visits and on-call support are provided. Patients can receive hospice care at home or in a skilled nursing center, and hospice care is also available at independent or assisted living communities or hospitals.
