Provide primary therapy from evaluation to discharge, delivering functional rehabilitation in real‑life settings such as home, school, work, and community.

• An experienced PT seeking a primary therapist role from evaluation to discharge
• Providing functional rehabilitation wherever life happens, whether at home, school, work, or in the community
• Treating clients beyond staged environments into real life experiences such as cooking, navigating grocery stores, restaurants, outdoor activities, and the workplace

• Perform evaluations and develop treatment plans
• Regularly reassess the effectiveness of treatment plans, attend staffing meetings, and family conferences
• Perform discharge evaluations, make recommendations for home and community safety, and provide referral and education resources
• Communicate patient needs and progress to the treatment team, physician, service recipients, and family members
• Communicate with other disciplines to ensure collaboration, coordination of care, and enhanced patient outcomes

• Minimum bachelor’s degree in Physical Therapy from an accredited program
• Current, unrestricted state license as a Physical Therapist
• Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations
• Preferable one year’s work experience as a physical therapist
• Knowledge of rehabilitation techniques related to complex neurological injury
• Effective professional communication, both verbal and written
• Ability to lift 50 pounds; ability to lift or transfer up to 100 pounds following safety procedures
• Fine motor skills and visual acuity; ability to walk and stand for extended periods
• Ability to push, pull, bend, stoop, reach, kneel, and position frequently
• Health screen or examination may be required prior to assignment and periodically thereafter, per location policy and regulations, to verify physical capability with or without reasonable accommodations
