Job Summary
The Social Worker position provides comprehensive, compassionate, clinical social work and discharge planning to patients and families of a targeted patient population on a designated unit(s). The role involves educating patients on treatment, health, and wellness choices while facilitating psychosocial services.
Responsibilities- Provide comprehensive, compassionate, clinical social work to patients and families.
- Conduct discharge planning for patients on the unit.
- Educate patients about treatment options, health, and wellness choices.
- Deliver psychosocial services to dialysis patients, both in-center and at home.
- Utilize Social Work Theory of Human Behavior and accepted methods to support patients.
