Monogram Health is seeking Advanced Practice Providers (NPs or PAs) to deliver in-home care for patients with multiple chronic conditions in Texas. You will perform comprehensive assessments, educate patients, and coordinate with PCPs and specialists to improve outcomes.
Travel to patient homes is required, with visits scheduled 8am-5pm Monday through Friday. Active licenses, board certification, and DEA are required, along with 2+ years of direct patient care.
