Job Summary: Integral Care's Expanded Mobile Crisis Outreach Team (EMCOT) is seeking a Community-Based Crisis Case Manager to provide comprehensive support to individuals transitioning from crisis stabilization to ongoing services. This role will focus on opening individuals to Level of Care 5 (LOC-5) Transitional Services, ensuring that the crisis has stabilized before connecting them to long-term care.
This position involves diagnosing, psychotherapy, case management, and living skills in a community-based setting.
The ideal candidate is flexible, detail-oriented, a team player, an effective communicator, calm under pressure, and committed to growth and learning.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:
Required:
- Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field.
License:
Required:
- LPC, LCSW, or LMFT certified.
- Current and valid driver's license and a clean driving record.
Experience:
- Experience in crisis intervention, case management, or community health preferred.
- Strong clinical assessment, problem-solving and communication skills.
- Ability to work in field-based settings and travel to clients as needed.
Hours: Full-Time.
Workdays: Varies.
Workhours: Varies/Flexible.
Salary:$74,297.27 - $94,721.12 based on years of licensure,plus an additional 10% stipend.
Position may be eligible for an additional shift differential pay.
- Second Shift: $3.25/hour.
- Third Shift: $7.25/hour.
