Brookdale at Home is looking for a Certified Nursing Assistant (CNA) to provide one‑on‑one care to seniors in their own homes. Flexible scheduling, part‑time, full‑time, and PRN positions are available with same‑day interview opportunities.
Responsibilities- Work independently in the client’s home under the supervision of a licensed nurse or the BAH Agency Leader as required by state regulations.
- Communicate changes in the client’s care needs to the BAH Agency Leader.
- Provide personal care and/or concierge, homemaker, or companion services per the individualized plan of care.
- Document all pertinent information regarding care.
- Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, transferring, and getting to and from activities and meals.
- Prepare clients for meals, assist to and from dining areas, and help set up and feed meals.
- Provide toilet and incontinence care, performing incontinence checks at least every two hours and providing care when required.
- Keep the client’s home neat and clean.
- Perform vital signs, blood pressure, weight, and other trained procedures as per the client’s care plan.
- Execute prevention and restorative care, including PROM/AROM, ambulation, transferring, bathing, grooming, and other ADL activities.
- Maintain knowledge of agency policies and procedures relevant to the role.
- Assist with turning, positioning, and repositioning of clients.
- Communicate effectively with clients, families, visitors, and agency staff.
- Participate in the Community Quality Assurance Program.
- Attend required ongoing education to maintain compliance with state and federal regulations.
- Maintain client confidentiality and HIPAA compliance.
- Assist with homemaker and companion tasks such as laundry, shopping, light housekeeping, and meal preparation.
- Follow infection control and safety policies and guidelines.
- Arrange and maintain reliable transportation to arrive on time for assignments.
Education and Experience
High school diploma or GED required. Must have completed an approved Certified Nursing Assistant program. Six months of independent experience with clients in their homes is preferred.
Certifications, Licenses, and Other Requirements
Successful completion of State CNA course with valid certification. Reliable transportation required. Valid state identification card; a driver’s license is preferred.
Physical Demands and Working Conditions
- Standing, walking, sitting, and use of hands and fingers.
- Reaching with hands and arms; climbing, balancing, stooping, kneeling, crouching, or crawling.
- Talk or hear; taste or smell.
- Lift up to 50 pounds.
- May be required to work occasional weekends, evenings, or nights.
- Possible exposure to communicable diseases, blood‑borne pathogens, latex, and various chemical or biological hazards.
- Travel to client homes frequently.
Management / Decision Making
Makes standard and routine decisions based on provided guidelines and reports issues to a supervisor.
Knowledge and Skills
Follows written or verbal instructions, basic technical skills for repetitive duties, and knowledge of facility emergency procedures. Able to maintain confidentiality and adhere to HIPAA policies.

Base pay will be based on skills and experience. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.
