The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. Essential Job Functions (EJFs)

(Beginning) specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

Ensures support is maintained for all individuals in their living environments by working with the Interdisciplinary Team (IDT) including but not limited to the Qualified Developmental Disability Professional, Nursing and Physicians, Home manager and staff, Habilitation and Vocational Therapies, parents/guardians, the individual, etc. to create design plans, themes, and color schemes that fit the individuals’ needs and personality.

Monitors living conditions and prepares and distributes correspondence, forms and documents and monitoring reports as needed for work orders and maintenance requests. Purchases items needed for design plan.

Participates as a team member to plan resident assessments and program goals, objectives, strategies, and activities. Designs and presents plan for the individual's environment, to include but not limited to: paint scheme, home décor scheme, bedding, furniture, window dressings, etc.

Communicates with other departments including but not limited to IDT, maintenance, purchasing department, volunteer services and other residence and facility employees or other interested parties regarding individuals’ home décor needs and services.

Maintains facility property, equipment, and individuals’ personal effects. Develops and maintains inventory on hand for each design plan for immediate replacement for damaged items.

Performs related work as assigned. May assist with facilities management responsibilities, including repairs, key controls, or supply and equipment requisitions.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
