As a Program Coordinator at Sevita, you will support home and community-based healthcare and social service programs that empower individuals and families. In this role, you'll coordinate daily operations, assist with person-centered service plans, and ensure high-quality, compliant care. You'll collaborate with clinical teams, families, and community partners, track outcomes, and support continuous improvement. Responsibilities

  • Coordinate day-to-day operations of community-based healthcare and social service programs
  • Support person-centered service planning and ensure timely, accurate documentation
  • Monitor program quality, compliance, and outcomes in line with Sevita and regulatory standards
  • Collaborate with families, caregivers, community partners, and clinical teams to support participants
  • Assist with scheduling, referrals, intakes, and resource linkage for individuals and families
  • Collect and report program data; support audits, quality reviews, and continuous improvement efforts
  • Provide outreach and education about Sevita's services within the community
  • Contribute to a positive, inclusive culture that reflects Sevita's values of empathy and collaboration
