As a Program Coordinator at Sevita, you will support home and community-based healthcare and social service programs that empower individuals and families. In this role, you'll coordinate daily operations, assist with person-centered service plans, and ensure high-quality, compliant care. You'll collaborate with clinical teams, families, and community partners, track outcomes, and support continuous improvement. Responsibilities
- Coordinate day-to-day operations of community-based healthcare and social service programs
- Support person-centered service planning and ensure timely, accurate documentation
- Monitor program quality, compliance, and outcomes in line with Sevita and regulatory standards
- Collaborate with families, caregivers, community partners, and clinical teams to support participants
- Assist with scheduling, referrals, intakes, and resource linkage for individuals and families
- Collect and report program data; support audits, quality reviews, and continuous improvement efforts
- Provide outreach and education about Sevita's services within the community
- Contribute to a positive, inclusive culture that reflects Sevita's values of empathy and collaboration
