Stellar Care Staffing is seeking a PRN Occupational Therapist (W2) tojoin our team. We believe in providing elite clinical support while giving our providers the freedom to balance their professional and personal lives.
As a PRN OT with Stellar Care, you will provide high-quality home health services to patients, helping them regain independence in the activities that matter most. You pick the coverage areas and the days you want to work—we handle the rest.
What You’ll DoRestore Independence: Conduct initial evaluations and develop goal-oriented plans focused on ADLs (Activities of Daily Living) and IADLs.
Clinical Excellence: Provide one-on-one therapeutic interventions, including fine motor coordination, cognitive retraining, adaptive equipment training, and activities within your scope of practice.
Environmental Mastery: Perform comprehensive home safety assessments and recommend modifications or specialized equipment to foster aging-in-place.
Documentation: Complete timely OASIS assessments and progress notes via our intuitive EMR system.
Collaborate: Work closely with physicians and the Stellar Care clinical team to ensure seamless patient transitions and holistic care.

RequirementsWhat You Bring
Credentials: A degree in Occupational Therapy from an accredited program, NBCOT certification, and a valid state license.
Experience: Minimum of 1 year of clinical experience (Home Health experience preferred).
Reliability: A valid driver’s license, auto insurance, and dependable transportation.
Tech-Savvy: Comfort with electronic documentation and mobile communication.
