Lifespan Home Health is seeking a Care Team Assistant II for our Austin TX Agency.

We are an established and stable home health agency that provides in-home, medical and non‑medical care to the elderly and people with disabilities so they can remain safe and comfortable in their home, with their family, and in the community.

• Responsible for the receipt of home care referral and the scheduling of visits as prescribed in the plan of care.
• Will receive referrals and telephone/facsimile orders serving as the communication center between physician case manager, staff members, and client.
• Responsible for informing inquirers about the organization’s services and about home care in general.
• Schedules or reschedules visits when staff members are unable to make them as scheduled.
• Collaborates with payroll department to ensure that employee’s time records are accurate.
• Conducts client admissions/evaluations with the exception of clients in the PHC program.
• Submits required reports within the assigned time frame.

• High school diploma or GED required.
• Six months to one year previous experience in a home health agency or other medical setting preferred.
• Travel required for client visits/hourly care, as necessary.
• Home health experience strongly preferred.
• Bilingual in Spanish preferred.
