We are seeking an experienced, licensed Speech Therapist to join our home health team on a PRN, per-visit basis! As a Speech Therapist, you will travel to patients' homes, meeting with new patients, assessing their speech needs, and creating personalized treatment plans to help them reach their goals. You will also document all patient care, observe and participate in treatment, and adjust goals and milestones as needed for the best possible outcomes.

The ideal candidate has a strong understanding of speech pathology, is comfortable working independently in a home setting, is able to provide compassionate care to every patient, and has excellent organizational skills.

• Meet with patients for the first time in their speech journey, in the comfort of their own home, answering introductory questions
• Make evaluations and create treatment plans customized for that patient, working with them to set short and long term goals for their speech and wellness
• Assist in the implementation of the treatment plan, adjusting as needed to ensure successful outcomes
• Maintain excellent records of patient needs, care, and outcomes, including timely documentation in the agency's EMR system for each visit
• Work closely with the family of the patients and the patients themselves to ensure they have the knowledge and tools they need for success
• Communicate patient progress and changes in condition to the interdisciplinary team and referring physician
• Complete recertification and discharge documentation as needed
• Maintain compliance with Medicare/Medicaid home health regulations and agency policies

• Active state license for practicing speech therapy
• Current Certificate of Clinical Competence (CCC-SLP) from ASHA, or eligible
• Strong communication and interpersonal skills
• Excellent organizational skills
• Strong charting and documentation skills
• Familiarity with basic computer programs, such as the Microsoft Office suite
• Home health experience preferred
• Reliable transportation, valid driver's license, and current auto insurance
• Current CPR certification
• Ability to pass background check and drug screening
