Full‑Time Physical Therapist – Home Health
We are looking for a registered Physical Therapist in South Austin, Texas. This full‑time position involves coordinated care to patients of all ages in a home‑health setting.
Key Responsibilities- Full‑time position
- Conduct ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs and determine level of functioning.
- Develop and evaluate the care plan in partnership with the patient and caregivers.
- Document, prepare, and complete clinical notes, phone orders, and other clinical record documentation in a timely manner.
- Provide services ordered by the physician as indicated in the plan of care; oversee other team members when applicable.
- Apply infection control and standard precautions during patient care.
- Accept clinical assignments consistent with education and competence; supervise PT assistants and home‑health aides as appropriate.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and assessment.
- Communicate with physicians and other health care practitioners related to the current plan of care.
- Meet mandatory continuing education requirements of the Agency and licensing board.
- Assist and instruct patients and families in transfer techniques; instruct on use of equipment such as walkers, wheelchairs, canes.
- Utilize appropriate therapy equipment specific to patients’ diagnoses and needs.
- Provide patient, caregiver, and family counseling and education.
- Use effective interpersonal relations and communication skills.
- Active state license as a physical therapist or a multi‑state license issued by a Physical Therapy Licensure Compact (PTLC) member state.
- 1 year of experience as a physical therapist in an acute care, rehabilitation, or home‑health setting is preferred. The client is willing to train.
- Therapy skills as defined by generally accepted standards of practice.
- Good interpersonal skills.
- Proof of current CPR certification.
- Knowledge of durable medical equipment.
- Experience with EMR;
Kinnser experience preferred but not required.
