Referral Coordinator

Position Summary

The Referral Coordinator is a licensed social worker responsible for supporting the referral and admissions process for Hospice Austin and Austin Palliative Care. This position conducts informational visits with patients, families, caregivers, and referral sources to explain hospice and palliative care services, eligibility requirements, and available support. The Referral Coordinator also provides assigned palliative care social work visits, completes timely documentation, coordinates with members of the interdisciplinary team, and helps ensure a compassionate and seamless transition into services.

Interpersonal Relationships

All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.

Essential Functions
  • Performs intakes and processes referrals.
  • Performs data entry (excel, Suncoast, Netsmart MyUnity EMR) and maintains records and information for patients being admitted to Hospice Austin.
  • Coordinates patient care.
Qualifications
  • Master’s degree in social work
  • Current Texas Licensed Clinical Social Worker (LCSW) license
  • Minimum of two years of hospice and palliative care experience
  • Skill in office computer software
  • Valid Texas driver’s license, auto liability insurance and safe driving record
Preferred Qualifications
  • Experience in hospice/home health admissions preferred.
