PositionSummary
An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite. The role ensures patient safety with continuity and compliance under a physician’s plan of care and works under structured supervision of the designated supervisor.
Job Responsibilities- Initiate, develop, and implement intermediate nursing plan of care treatments, evaluate patient progress toward goals, and modify the plan of treatment in response to changing patient status or physician orders.
- Assess patient needs and physical status at each skilled visit, conduct physical reassessments, and clarify physician orders for plan of treatment revisions.
- Provide training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
- Teach patients and families using a patient‑centered approach to promote independence with prescribed therapy.
- Communicate effectively and promptly with Option Care’s Clinical team, medical providers, patients, and families to facilitate continuity of care.
- Prepare and submit clinical documentation in real time during visits to meet billing optimization timelines.
- Coordinate discharge planning and prepare discharge summaries with patient instructions and reports when transferring care.
- Demonstrate compliance with agency operations, policies, professional standards, and all applicable regulations and accreditation standards.
- Maximize work efficiency through the use of computers and other automation technologies.
- Respond promptly to patient requests and initiate emergency procedures as necessary.
- Accept accountability for own practice through ethical and professional conduct.
- Observe legal and ethical guidelines to safeguard patient confidentiality, including HIPAA regulations.
- Explain Option Care’s services and financial obligations to patients clearly.
- Attend and complete training modules, in‑services, and continuing education to maintain competency and licensure.
- Provide oversight and coordination of paraprofessionals and act as a supervisor when the Supervisor or Nurse Manager is absent, following state and federal regulations.
- Participate in multidisciplinary team conferences and mentor other nurses for orientation, onboarding, and supervisory activities.
- Participate in nursing department on‑call responsibilities.
- Perform other related duties as directed by supervisor.
- Insert PICC/Midlines per physician order.
- Troubleshoot/decolog obstructed central line catheters.
- Monitor PICC/Midline and educate on prevention of line infection and routine care.
This position does not have supervisory responsibilities.
BasicEducation and Experience
- Active and unrestricted RN license in the state of practice.
- Minimum of 1 year of nursing experience.
- Current CPR certification required.
- Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs; advanced certification and training as applicable.
- Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel and proof of liability insurance.
- Basic knowledge of computer operating systems and software applications, and ability to apply nursing technology tools to communicate and document care.
- Ability to lift up to 50 pounds with a maximum of 75 pounds; exerting up to 20 pounds of force occasionally, or up to 10 pounds frequently.
