Job Summary

The Nurse Practitioner (NP) provides holistic, compassionate, and patient‑centered care to adult and geriatric Members within skilled nursing and long‑term care settings. As a full‑time Nurse Practitioner, you will collaborate with physicians, RNs, case managers, and interdisciplinary care teams to diagnose, manage, and prevent acute and chronic conditions while promoting wellness and quality of life. This role includes accountability for Plan Medical Management, comprehensive health risk assessments, chronic disease management, and care coordination.

The NP practices within the scope of state regulations, clinical protocols, and collaborative practice agreements, with a strong focus on post‑acute care, provider‑based care, and value‑based clinical performance.
