We're seeking an experienced and compassionate RN to join a growing Home Health team serving patients throughout the Austin area.

This role offers the opportunity to provide one-on-one care, help patients maintain their independence, and become a trusted resource for patients and families in the comfort of their homes.

** What You'll Do*
* + Perform comprehensive patient assessments and develop individualized care plans

+ Provide skilled nursing care in patients' homes

+ Educate patients and caregivers on disease management, medications, and treatment plans

+ Monitor patient progress and communicate changes in condition

+ Coordinate care with physicians, caregivers, therapists, and interdisciplinary team members

+ Complete timely and accurate clinical documentation

+ Support patients in maintaining their health, safety, and independence at home

** Qualifications*
* + Active and unrestricted RN license

+ Home Health experience preferred

+ OASIS experience strongly preferred

+ Current CPR certification

+ Strong clinical assessment and critical thinking skills

+ Excellent communication and patient education abilities

+ Ability to work independently and manage a field-based schedule

+ Valid driver's license and reliable transportation
