The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, according to developed care plans and physician orders for each patient while maintaining confidentiality and professional boundaries.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs, reporting changes to supervisor or physician.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participate in, implement, and update the nursing care plan.
- Take appropriate nursing action based on assessment to achieve expected outcomes.
- Recognize changes in patient needs or responses that require intervention and implement care to prevent or reduce risk.
- Maintain personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Employ infection control measures that protect staff and patients per OSHA standards.
- Educate patient and family regarding disease processes, self-care techniques, and prevention strategies.
- Stay current on nursing competencies through education, continued learning units, and skill evaluations.
- Graduate of an accredited nursing school.
- Current, unrestricted state license as an LPN/LVN.
- Current CPR certification.
- Proficiency in clinical assessments, documentation, and compliance with nursing standards.
- Active, unencumbered LPN/LVN license in the practice state; compact licenses transferred within 90 days.
- Current TB skin test within last 12 months.
- Reliable transportation.
- Preferred:
Six months of recent experience as a Licensed Nurse in a clinical setting; home health experience. - State-specific requirements (e.g., South Carolina – 1 year pediatrics experience; California – 1 year experience under current license; Louisiana – 1 year as licensed nurse).
- Continuing education as required by state.
- Speak, write, read, and understand English.
- Travel as needed.
- Lift up to 50 pounds and reposition patients and equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Sit, climb stairs.
- Visual and hearing acuity; strong sense of smell and touch.
- Respond physically and mentally to emergency situations in home or transport.
- Function in varied environments with possible exposure to allergens, blood, bodily fluids, infectious diseases.
