- Lead the implementation and continuous monitoring of personalized patient care plans to ensure every individual receives treatment tailored to their specific recovery goals.
- Coordinate multidisciplinary teams as the primary point of contact, ensuring seamless communication across all clinical specialties for holistic patient support.
- Advocate for patient safety by diligently executing physician orders and maintaining precise records of the care journey to uphold the highest standards of quality.
- Assess and facilitate comprehensive discharge strategies, collaborating with the healthcare team to ensure patients have a safe, well-supported transition home.
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
Education:
- Diploma from an accredited school/college of nursing required OR a bachelor's level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
- Education & Language:
Holds a BSN from an accredited institution, with bilingual proficiency in Spanish preferred. - Clinical
Experience:
Offers 4 years of registered nursing experience, including triage and pediatric catheterization.
