** What You'll Do*
* - Provide skilled nursing visits in accordance with physician orders and established care plans

- Assess patient needs and document clinical findings accurately and thoroughly

- Maintain complete and compliant medical records

- Educate patients and families regarding treatment plans and ongoing care needs

- Communicate effectively with physicians, caregivers, and interdisciplinary team members

- Monitor patient progress and report changes in condition as appropriate

** Qualifications*
* - Graduate of an accredited Practical Nursing or Vocational Nursing program

- Current Texas LVN license in good standing

- Minimum of one year of clinical nursing experience

- Minimum of one year of home health experience
